Sample Written Summary Forms

Summary data can be submitted to the CBC-Monitor QC Program using these concise forms.  For each parameter, the 'N' value or number of control samples run, the Mean value and the Standard Deviation or SD is filled in to complete the form.  An added feature is that this form may be faxed directly to R&D Systems.
